Shot Records
Sweetgrass Pediatrics does NOT require a Medical Information Release Form as Shot Records are public information; and in compliance with the Health Insurance Portability & Accountability Act (HIPAA). There is no fee for shot records.
Complete Medical Records
To request complete medical records, click on the link below to the online request form. If requesting records for more than one child, submit a separate request for each child. There may be a fee for processing of records as defined in SC State Law Section 44-7.130.
Patients 18 years or older must complete their own release. Visit summaries are available in your patient portal, if you would like to request access to your patient portal please text us @ 843-416-3333.
