Complete Medical Records
To request complete medical records, please complete the Medical Records Release form linked below. If requesting records for more than one child, please submit a separate request for each child. There may be a fee for processing of records as defined in SC State Law Section 44-7.130.
Patients 18 years or older must complete their own release.
