Medical Records Requests
To request complete medical records, please complete a Medical Records Release form linked below. If requesting records for more than one child, please submit a separate request for each child. Patients 18 years or older must complete their own release.
There may be a fee for processing of records as defined in SC Code § 44-115-80.
SHOT RECORDS
Need Your Child’s Shot Record?
For children in kindergarten through 12th grade who meet school vaccination requirements, families can download their child’s school immunization certificate through the South Carolina Department of Public Health’s SIMON portal. No phone call or office visit is needed.
Follow the portal instructions to verify your identity, then select “Download School Certificate” for school enrollment or “Download Full Record” for a personal vaccination history.
Need Daycare Records or Help?
For daycare certificates or help obtaining your child’s shot record, text “SHOT” to 843-978-2349 or call your nearest Sweetgrass location. A Medical Records Release form is not needed for shot records alone.
Requests handled by our nursing team may take up to five business days, or longer during periods of high demand.
