Medical Records Requests
To request complete medical records, please complete a Medical Records Release form linked below. If requesting records for more than one child, please submit a separate request for each child. Patients 18 years or older must complete their own release.
There may be a fee for processing of records as defined in SC State Law Section 44-7.130.
SHOT RECORDS
Requesting Vaccination Records?
A medical records release form is only required for complete medical records. For vaccination (shot) records, please text us “SHOT” at 843-416-3333. You may also request shot records by calling our office. Please note that it may take up to 5 business days to complete your request, as all records must be reviewed by our nursing staff. Processing times may be longer during periods of high demand.
