Getting started with Sweetgrass is simple.

Joining Sweetgrass Pediatrics is designed to be straightforward for busy families. From completing your child’s registration to scheduling your first visit, we’ll walk you through each step and help coordinate any records needed from your previous provider. Our goal is to make your transition seamless so you can focus on your child’s care.

    Complete Patient Registration

    Start by filling out our secure online registration form for each child. This gives our team the information needed to create your child’s medical record and begin the onboarding process. Once submitted, our staff will review your details and reach out with next steps.

    Transfer Medical Records

    If your child has been seen by another pediatrician, we’ll help coordinate the transfer of medical records. This ensures your provider has a complete view of your child’s health history, including immunizations, past visits, and any ongoing care needs.

    Schedule Your First Visit

    After registration is completed and accepted by our team, and you’ve initiated a records transfer, you may schedule your child’s first appointment. Appointments can be scheduled online, via text message, or by telephone. Prior to your visit, you’ll receive a convenient pre-check-in form through Phreesia.

    In most cases, we’re able to schedule new patient visits within a day of pre-registration being completed. New patient appointment availability may vary depending on your preferred location and provider availability, but we’re usually able to see you within a week or two. Our team will work with you to find a convenient time and ensure your child is seen as soon as possible—especially if there are more immediate care needs.

  • Can I schedule online as a new patient?

    New patients begin with our online registration process rather than direct scheduling. However, you will be able to make your first appointment online following completion of new patient registration and confirmation by our team! Registration review is important, as this allows us to gather important information and ensure your child is scheduled appropriately.

  • What if my child needs care before our first scheduled visit?

    If your child needs care before your initial appointment, please call or text our office. We’ll do our best to accommodate urgent needs or guide you to the appropriate care option. In many cases, we can help facilitate a same-day visit within our network, depending on availability.

  • How do I transfer my child’s records from another pediatrician?

    After completing your registration, our team can help coordinate collection of your child’s medical records from your previous provider. The easiest and fastest way to get started would be to contact your child’s previous provider’s office and request a record transfer. Having records available before your first visit helps ensure a seamless transition and continuity of care.
